Department of Transportation Physicals (DOT)

To help speed up your DOT and clearance process, please review the chart below in advance. Use the link provided to access the necessary form and take it to your treating physician before your Occupational Health appointment.

Our goal is to ensure you’re cleared quickly and able to stay on the road. Staying compliant with your treatment plan and completing all required forms will help prevent delays. Be sure to bring all requested documentation with you to your physical appointment.

Cardiac Clearance
If you see a cardiologist, we require a current letter from them stating that you are cleared to drive commercially.

Sleep Apnea
If you have sleep apnea, you must bring a 90 day CPAP compliance report with you for your physical. It is required to show consistent use of a CPAP machine and compliance with your treatment plan.

Insulin Use
If you are prescribed insulin, you must have the following paperwork filled out by the provider who manages your diabetes.

If you have monocular vision, you will need to have the following form filled out by your treating ophthalmologist before your visit.

Some medications require close monitoring for DOT drivers. If you take controlled substances, psychiatric medication, or neurological medications, please have your treating physician fill this form out before your visit.
